Requesting a layout

i will only take requests if you fill out the form properly. all other requests will be ignored and deleted.
you can send requests to my witty layouts user; Speechless_layouts, under comments.
i will comment back and tell you if your request is okay (meaning their will be no problems with making it.)
your layout will be sent to you within two weeks after i comment back.
when your layout is finished, i will post a picture of it on the "previews" page so you can see how it came out and tell me what you would like change before you put it in your profile.
to add to your profile, follow the instructions given under "premade layouts" -> "how to use.

send request in COMPLETE AND CORRECT format. i will not accept any requests that aren't in proper format.
DO NOT remove credit. EVER. I took time out of my already busy enough life to make a layout special for you. dont take my credit.
DO NOT copy someone elses exact request. not gonna happen.
NO NOT send me any more comments rushing me. (ex. "is my code done yet?" "when will it be done?" "hurry up") i'll give it to you when im finished. if you do this, i will delete your request and any part of a layout i've already made, and put you on the list of people to not make layouts for ever again.
if you have already requested a layout from someone else, do not ask for one from me and decide which one you like better. i can promise i will never make another layout for you. ever.
