to submit a request, you need to fill out this form COMPLETELY and CORRECTLY and post it under comments on my witty user Speechless_layouts.

- give me the URL of any specific picture(s) you have already picked out, if not, give me a general category for the picture(s) you want.
COLORS
- any color theme you would like me to use?
- or you can ask for me to match the colors within the picture
HEADING
- do you even want one?
- if so, be specific. do you want it to say your username? do you want anything else to be included?
QUOTES
- do you want a quote on it?
- if so, please tell me what you want. if you want lyrics, tell me exactly what part of the song you want.
- it doesnt have to be a quote, it can just say "i love summer" or "save the turtles" or anything. tell me whatever you want it to say.
TEXT BOXES
- how many do you want?
- what headings should they have?

- any specific other than just arial?

- if you do not fill out any of the categories or put "i dont care" i will choose for you. and i will not go back and change it if you dont like it.
